Appreciate it if you can help me with the survey for the research paper.

I am conducting a survey on approaches that a Supply Chain Department is using to manage performance. Really appreciate it if you could help to fill in the questionnaire if you are in the Supply Chain functions (Procurement, Inventory, Logistic, Material Planning etc).

Would appreciate it as well if you could help to forward this to your friends who are in the Suply Chain function. Thanks a lot for your help.

* Due to the sensitivity of the questions and to protect the anonymity of the respondents, I have set up the questions in a website thus I will not know who the respondents are as well as making it easier to answer.
